He was beaten to death by orderlies after becoming violent.
His vengeful spirit now chases players down dark hallways.

4. The Mirror Mask Chapter – The Truth About Dwayne

This final chapter reveals Dwayne’s own dark past.
He was a father struggling with drug addiction and depression.
Under the influence of hallucinogenic drugs, he murdered his wife and child, believing it was the only way to “save” them.
Overcome by guilt, he shot himself, but instead of dying, he became trapped in the haunted house—a purgatory of his own making.
